2. Your Mission: The Role of a Care Management Associate

As a Care Management Associate within our Aetna division, you will be a crucial navigator in the healthcare system for our members. Your primary mission is to support the comprehensive coordination of medical services, acting as a key point of contact and support. You will be the foundational support for our Care Team, ensuring that members receive the right care at the right time.

3. A Day in the Life: Your Key Responsibilities

Your daily activities will be dynamic, impactful, and centered around member support. Key responsibilities include:

  • Care Team Intake and Screening: You will be responsible for the initial stages of the care management process, conducting member screenings and gathering essential information to help the Care Team understand each member’s unique needs.
  • Care Plan Implementation Support: You will work closely with nurses, social workers, and other healthcare professionals to support the execution of personalized care plans, ensuring members understand and can follow their prescribed health pathways.
  • Comprehensive Service Coordination: Your role involves coordinating a wide range of medical services, from scheduling follow-up appointments to assisting with referrals and connecting members with necessary resources.
  • Member Communication and Support: You will serve as a telephonic liaison for members, answering questions, addressing concerns, and providing proactive support to promote positive health outcomes.
  • Data Management and System Navigation: You will utilize internal and external computer systems to document interactions, update member records, and track the progress of care plans, ensuring all information is accurate and up-to-date.
  • Multi-Disciplinary Collaboration: You will effectively participate in a team that includes both internal colleagues (like nurses and pharmacists) and external participants (like physicians and clinic staff) to provide a seamless care experience.

Required Qualifications:

  • Experience: 2-4 years of relevant experience as a medical assistant, clinical office assistant, or in a similar healthcare administration role.
  • Technical Proficiency: Must possess strong computer literacy with the ability to navigate through multiple internal and external computer systems. Proficiency in Microsoft Excel and Word is required.
  • Education: A High School Diploma, G.E.D., or equivalent higher education is required.
